Oak Tree Homes is a licensed Short-Term Residential Therapeutic Program (STRTP) in Nuevo, California. Our program is designed to help youth stabilize, build coping skills, and move toward a less restrictive, family or community-based setting.

HOW WE APPROACH CARE
A plan built around
each young person.
Our program statement brings together clinical treatment, daily support, education, and family connections. Services are individualized to assessed needs and coordinated with the youth’s care team.
Individualized treatment
Assessment and treatment planning consider strengths, needs, cultural identity, and personal goals. The program model includes individual, group, and family-focused treatment, with the approach selected by qualified clinicians.
Skills for everyday life
Coping strategies, emotional regulation, communication, healthy relationships, and daily living skills connect treatment goals with everyday experiences.
Education and coordination
The program model includes school participation and coordination with educational, medical, behavioral health, and county partners so support can work together.
FAMILY & TRANSITION
Planning for what
comes next.
Youth and their Child and Family Team help shape goals and review progress. Family members, caregivers, and other important supports participate as appropriate to the youth’s needs, permissions, and care plan.
Transition planning connects the next placement with ongoing mental health, education, and community supports. Readiness is reviewed individually; the goal is a timely move to the least restrictive appropriate setting.
